Getting started is simple!
Just follow the instructions below for each patient:

Fill out and sign a prescription including ICD-10 Code (pads will be enclosed).

Complete a Letter of Medical Necessity and sign (forms will be enclosed).
3
Provide notes and patient demographics
For the first two months, all you need is getting the prescription (step 1) ready. We’ll come in weekly/biweekly and fill out and prepare steps 2 & 3.
